Build a consistent quality score for presentation checks and keep non-applicable answers out of the total.

Overview

Use scored answers when an audit rates presentation quality across areas such as entrances, signs and guest facilities. This example uses the same scale for each check so the final percentage is easy to explain.

Before you start

Agree the criteria for each answer before assigning points. A score does not certify safety or automatically change operational readiness. Read the scoring calculation if you plan to weight questions differently.


Walkthrough

1. Use a single-choice scale

Add a Select Question and choose Single choice. Create clear options such as Meets standard, Partly meets standard and Needs attention. Describe the standard in Explanatory Text.

2. Assign consistent points

Set Points to 2, 1 and 0 for those three answers. Reuse the same scale on equally important checks. Zero is a scored failure; an empty points field excludes that answer from the score.

3. Handle non-applicable checks

Where a check can genuinely be inapplicable, add N/A and leave its Points field empty. Explain when staff may use it. Do not use N/A to record a check that should have been performed but was missed.

4. Review a sample Result

Save, complete and sync a test audit. Open the Result report and compare its answers, page scores and overall score. For two equally weighted checks answered 2 and 1, expect 3 out of 4 possible points, or 75%.


Check the outcome

The test percentage matches the scoring scheme. A failed check records zero, while an allowed N/A answer is excluded from the possible total.

  • Treat changes to weights as a reporting change.

Frequently asked questions

Why can’t staff reach 100% on a multiple-choice question?

Its maximum includes the points of all options. Use Single choice when exactly one rating should apply.

Does a missed Checklist count as zero?

No. An uncompleted Checklist has no answer score. Some score widgets have a separate option to include unanswered Checklists as zero.

Does approval change a low score?

No. Approval does not change the recorded answers or score. See how scores work.
